We offer to obtain estimates of services from your insurance company as a courtesy to you. These estimates are not a guarantee of patient responsibility. To be able to look up this information, we need a front and back copy of your insurance card, your ID, and the name and DOB of the patient and subscriber. Those can be sent to info@tulsametropsych.com. Any same day cancellations are subject to a late cancellation fee, so please get your information to us 3-5 business days before your appointment.
